Vestibular Disease and Cats

Have you ever noticed your cat tilting their head to one side (similar to the old RCA dog logo)? If so, they may be experiencing a transitory problem with their vestibular system: “a complex arrangement of nerves and other components that governs its sense of balance and coordinates the movement of its head and eyes.”1 “If this system malfunctions, this is known as ‘vestibular disease’ (also known as ‘vestibular syndrome’), and it results in an inability for the cat to move as smoothly as normal. It is fairly common in cats, and results in our usually agile pets moving in an unbalanced and uncoordinated way.”2 “This disorder and its alarming manifestations are typically temporary and ultimately harmless. In some cases, the signs of what is known as vestibular disease may be due to a more serious problem.”1

“The vestibular system is divided into two sections which work together. The peripheral vestibular system is made up of receptors located next to the cochlea in the inner ear. The central vestibular system, located in the brain and spinal cord, is made up of three parts: the cerebral cortex (forebrain), cerebellum, and the spinal cord. Any condition which disrupts any of these components of the vestibular system can cause vestibular disease.”2

“When vestibular dysfunction occurs in dogs or cats it is most often associated with the peripheral system (inner ear) rather than with the central system (brain).”3

Conditions that can hasten vestibular system malfunction can range vastly in severity. This can include “bacterial infections; inflammatory disease; adverse reactions to certain drugs, including some antibiotics; and a variety of growths such as polyps, tumors, cysts, and cancer. In most cases, however, the cause of vestibular malfunction will remain unknown and may be referred to, therefore, as idiopathic vestibular syndrome.”1

Causes

“Vestibular disease can affect cats of all ages. Siamese and Burmese cats have been associated with an inherited or congenital form of the disease. Many affected cats are also deaf. Conditions such as middle- and inner-ear infections are common causes of the disease. Tumors are a less common cause but must be considered, especially in older cats. Exposure to certain toxins and drugs can also cause symptoms that mimic feline vestibular disease. Most cases are idiopathic, meaning the exact cause is unknown.”4

Common causes include:

“Otitis media (inflammation of the middle ear) and otitis interna (inflammation of the inner ear), nasopharyngeal polyps, Feline Infectious Peritonitis (FIP), meningitis or encephalitis (inflammation of the central nervous system), vascular infarcts (blood clots) or hemorrhage, congenital malformations of components of the vestibular system, and inherited CNS storage diseases. These are more likely to be seen in certain breeds of cat, including Burmese, Siamese, and Tonkinese, where the age of onset is usually when they are just a few weeks old.”2

Other less common causes are “cancer (tumors), general systemic diseases (including hypertension), thiamine deficiency and toxins (including some medicines), and head or spinal trauma which can cause direct physical damage to components of the vestibular system.”2

Symptoms

The most common medical symptoms of vestibular disease “include circling or falling to one side, a pronounced head tilt, and nystagmus – the rapid and involuntary oscillating movement of the eyeballs. Facial drooping may occur if there is a tumor or inflammatory disease of the inner or middle ear because the facial nerves are closely associated with the middle ear, which is next to the inner ear.”1

Normally a cat moves in a coordinated way; however, an infected cat will lose this ability and will start to become uncoordinated and lose their sense of balance. “They may also feel very nauseous, affecting appetite. This combination of factors presents as these symptoms: head tilt, abnormal position of the head, general weakness, leaning to one side, falling over to one side, staggering, incoordination of the limbs, rolling over and over, nystagmus (eyes flicking rapidly from side to side, or up and down) and facial paralysis.”2

Diagnosing

Unfortunately, there are no specific tests for the disease. Most cases are diagnosed based on medical symptoms. “Tests such as magnetic resonance imaging (MRI), skull radiographs (X-rays), blood and urine tests, spinal fluid analysis, and ear cultures and cytology (examination of any discharge or fluids found in the ear canal) are typically performed to rule out a more severe cause.”4 After considering the cat’s medical history, clinical signs, and physical examination findings, the veterinarian will determine which tests to proceed with. Additional tests may be required including “specific tests for some viral infections such as FeLV and FIV as well as checking the blood pressure. Most cats do recover, and ongoing medication is not needed.”2

Treatment

Treatment of vestibular disease depends on the cause. Hospitalization for fluid therapy and medication is common. “If the condition is secondary to infection, tumor, or toxicity, the primary disease must be treated.”1 “Antibiotics or anti-fungal medications will be used in middle- or inner-ear infection cases.”4 Since the cause of idiopathic vestibular disease is often unknown, there is no specific course of treatment.

“In idiopathic vestibular disease, treatment is focused on reducing motion sickness, nausea, and vomiting. Idiopathic vestibular disease is a short-term, self-resolving condition. Signs are usually the worst during the first 24-48 hours, then steadily improve. It does not continue for months. Usually, uncontrollable eye movement disappears within days. Older affected dogs can have a residual, often permanent, head tilt, but this rarely occurs in cats.”4

The veterinarian may prescribe medication to reduce nausea. “Maropitant citrate (Cerenia) is the most common drug to be used. Your cat may also be given anti-inflammatory medication. More specific treatments will depend on the diagnosis, but could include antibiotic treatment and surgical drainage of the middle ear in cases of otitis media or surgical removal of tumors. Many cats require assistance eating and drinking during the initial stages of the condition when their balance is most affected.”4

Due to a multitude of possible aspects that can affect individual cases, it is hard to estimate treatment costs. Before proceeding with any treatment, it is important to get a detailed estimate. If you have pet insurance, check to see if they cover this. “Costs could vary from $400 for a simple case that does not require significant intervention to $6,000 or more for an exceptionally complex case (e.g., with MRI scans and surgery of the central nervous system).”2

Prognosis

The recovery depends on the root cause. “Cats with idiopathic vestibular disease, otitis media/interna, and cerebrovascular disease often begin to improve within a few days, gradually recovering over two or three weeks. In some cases, certain signs (such as head tilt and facial paralysis) may remain permanently.”2 Other causes, “such as tumors, have a much more guarded prognosis.”4

Your cat may need help at home. “Cats should be supported in an upright position when eating or drinking to help them to eat, and to reduce the risk of aspiration of food or fluid. Keep rooms brightly lit since a cat’s vision may be impaired by the condition.”2 Reduce stress since it can worsen symptoms. Also make sure that the cat has a clear path around the home.

“Feline- and canine idiopathic vestibular disease are not life threatening. Most pets with good general health will naturally adapt and compensate such that symptoms begin improving within about three days and almost completely resolve in two weeks, although a head tilt may remain.”3

“Avoid carrying your pet. In the same way that a human with a vestibular disorder needs to move about to help recalibrate sensory information, your pet needs to retrain his system by navigating on his own. The touch sensors in a pet’s paws provide useful sensory information about balance when he walks or runs but they won’t be activated if his paws are dangling in air. In domesticated pets, peripheral vestibular dysfunction often results in severe and disconcerting behavior changes.”3 However, with proper veterinary care and home treatment, the condition can often be resolved.

“There are no specific ways to prevent vestibular disease in cats. Prompt treatment of ear infections can reduce the likelihood that the problem will spread to the middle or inner ear.”2 Annual vaccinations and check-ups are a good idea.
